SLO Forms
The following are the forms that are needed to submit SLOs and assessment cycle reports. Course and Program-Level SLOs are reviewed by the Valley College Curriculum Committee and then forwarded to the Senate. All other SLOs and all assessment reports are reviewed by the Outcomes Assessment Committee (OAC).

Discipline Assessment Plan (DAP) Form - submitted through the Annual Plan process
