MONARCH WINTER CAMP 2013-14
REGISTRATION INFORMATION

WE PREFER THAT YOU REGISTER IN PERSON AT THE COMMUNITY SERVICES OFFICE (located in the Field House on Ethel Avenue) M-TH 8 AM-5:30 PM and Friday 8 AM-4 PM. However, if you wish you may regsiter by phone or on-line.

To register for winter camp 2013-14 by phone or on-line please follow these steps.

1. Download and complete the following forms:

Registration Card

Waiver Form

2. Fax the forms to: 818-947-2930

3. Call us at 818-947-2577 x. 4172 to ensure that we have received the forms, select the week(s) your child(ren) will attend and to provide a credit card number as form of payment. OR pay on-line by clicking here (please note registrations are not considered complete and no spot will be held until both the waiver form and the registration card have been received in our office)

PLEASE NOTE: YOU MUST REGISTER AND PAY BY 5 PM THE TUESDAY PRIOR TO YOU CHILD ATTENDING CAMP. NO EXCEPTIONS!
