YOU MAY MAIL OR FAX YOUR COMPLETED FORMS TO US ONLY IF YOU ARE REGISTERING ON LINE FOR ANY OVERNIGHT ADVENTURE CAMP, WINTER CAMP, OR ANY SUPERSAVER PACKAGE.* 

 YOU MUST REGISTER IN PERSON FOR ALL OTHER PROGRAMS.

Regular registration hours are from 8:00 am to 5:30 pm, Monday through Thursday at the Community Services Office and on Fridays from
8:00 am to 12:00 noon. The Community Services Office is located in the Field House on Ethel Avenue across from the pool.

PLEASE NOTE:

REGISTRATION IS NOT FINAL UNTIL ALL FORMS ARE COMPLETE AND ON FILE AT THE COMMUNITY SERVICES OFFICE. YOU MUST COMPLETE NEW FORMS FOR EACH CAMP PROGRAM EACH YEAR (I.E. FORMS COMPLETED FOR SUMMER CAMP ARE NOT VALID FOR WINTER CAMP, ETC.).
 
For On-Line Registrations:
By registering on line you agree that you are aware that even though payment has been made in full registration will not be complete until we have a completed and signed Parent Card and Permission To Treat/Field Trip Waiver Form on file. In the event that you do not complete the parent card prior to all spaces in the camp being filled you will receive a refund of fees paid LESS $35.00 processing fee.

* THE COMMUNITY SERVICES DEPARTMENT IS NOT RESPONSIBLE FOR LOST OR MISDIRECTED MAIL AND WE STRONGLY RECOMMEND THAT YOU BRING THE FORMS IN PERSON TO OUR OFFICE, OR CALL US TO ENSURE THAT WE HAVE RECEIVED YOUR MAILED-IN OR FAXED-IN FORMS.
